Abstract
Severe fever with thrombocytopenia syndrome (SFTS) is a tick-borne infectious disease. A 91-year-old woman was admitted to our intensive-care unit with SFTS, and she developed dyspnea with wheezes 5 days after admission. Bronchoscopy showed scattered white mold in her central airway. An airway tissue biopsy and culture of bronchial lavage fluid revealed fungal hyphae in the necrotic tissue, confirmed as Aspergillus fumigatus. In this study, paddy was stored at different temperatures and relative humidity, and severe mildew was observed in samples stored at relative humidity of 97% and different temperatures (15, 28, and 37 °C) after 50 days. High-throughput 16S rRNA and ITS genes amplicon sequencing analyses showed that Proteobacteria, Firmicutes, and Ascomycota were the dominant phyla across the paddy samples. We describe a case of isolated acute appendicitis due to Aspergillus carneus in a neutropenic child with acute myeloid leukemia (AML) treated according to the AIEOP AML 2002/01 protocol. Despite prophylaxis with acyclovir, ciprofloxacin and fluconazole administered during the neutropenic phase, 16 days after the end of chemotherapy the child developed fever without identified infective foci, which prompted a therapy shift to meropenem and liposomial amphotericin B.
